DIRECTIONS TO PURCHASE BY MAIL

First, open the file by clicking on the link.

Second, you then may print off the document or simply go to 'FILE" and then "SAVE PAGE AS..."

NOTE: It is suggested that you make a photocopy of the receipt before mailing it in.

Second, please make out all checks or money orders to: Midwest Rampage, LLC

Third, mail your form and payment to: Midwest Rampage

Be sure to include any artwork, logos, ads, etc. with your mail or you may send your design via email to: Sales Department.

You will receive a confirmation email after your form and payment have been received.
